These terms apply to your use of serenityha.net. By using this site or submitting a form on it, you agree to them. If you do not agree, please do not use the site.

No medical advice

The content on this site is general information about the conditions we treat and the services we offer. It is not medical advice, a diagnosis, or a treatment recommendation, and it is not a substitute for evaluation by a qualified clinician. Do not start, stop, or change any treatment based on what you read here.

Emergencies

This website and its forms are not monitored around the clock and must never be used to report an emergency. If you are having thoughts of harming yourself or someone else, or are experiencing a medical emergency, call or text 988 for the Suicide and Crisis Lifeline, call 911, or go to your nearest emergency department.

No clinician-patient relationship

Browsing this site, sending us a message, or requesting a consultation does not create a clinician-patient relationship. That relationship begins only after we accept you as a patient, you complete our intake and consent documents, and a clinician evaluates you.

Consultation requests and scheduling

A submitted form is a request, not a confirmed appointment. Our staff will contact you to verify eligibility, discuss availability, and schedule. We may decline a request if the care sought is outside our scope, if we are not accepting new patients, or if you are located outside the states in which our clinicians are licensed.

Information you provide must be accurate and must be your own, or that of a minor or other individual you are legally authorized to act for.

Communications

By providing your phone number and email address, you agree that we may contact you by phone, text message, and email about your inquiry, appointments, and care. Message frequency varies, and message and data rates may apply. Reply STOP to opt out of text messages or HELP for help. Consent to text messaging is not required to receive treatment.

If you opt in to our newsletter, you will receive it no more than once a month and can unsubscribe from any message. Email and web forms are not secure channels. Do not use them to send sensitive clinical information.

Telehealth

Virtual visits are offered through a secure video platform to patients physically located in a state where the treating clinician is licensed. Telehealth requires a separate informed consent, which we review with you before your first virtual visit. Some services and evaluations must be conducted in person.

Insurance, fees, and cancellations

Coverage, copays, deductibles, self-pay rates, and our cancellation and no-show policy are described in the patient agreement you receive at registration. Nothing on this website is a quote or a guarantee of coverage or cost.

Intellectual property

The text, layout, graphics, photographs, and logos on this site belong to Serenity Health Associates or are used with permission. You may view and print pages for your personal, non-commercial use. Any other copying, distribution, or use requires our written consent.

Third-party content and links

Some features of this site are provided by third parties, including our appointment form, patient review widget, and educational material about specific treatments. Those services operate under their own terms and privacy policies. We link to outside resources for convenience and do not endorse or take responsibility for their content.

Accessibility

We work to keep this site usable for visitors with disabilities. If you encounter a barrier, call us at (978) 725-4800 and we will help you get the information you need and work to correct the issue.

To the fullest extent permitted by law, Serenity Health Associates and its clinicians and staff are not liable for indirect, incidental, consequential, or punitive damages arising from your use of this website, or from any inability to use it. Nothing in these terms limits liability for the care we provide to our patients, which is governed by applicable Massachusetts law and professional standards.

Governing law

These terms are governed by the laws of the Commonwealth of Massachusetts, without regard to its conflict-of-laws rules. Any dispute arising from the use of this website will be brought in the state or federal courts located in Essex County, Massachusetts.

Changes to these terms

We may revise these terms at any time. The effective date at the top of this page shows the current version, and continued use of the site after a change means you accept the revised terms.
