This policy explains how Serenity Health Associates collects, uses, and protects information you provide through serenityha.net. It covers our website and online forms. The privacy of your medical record is governed separately by federal and Massachusetts health privacy law and by our Notice of Privacy Practices, which we provide to every patient.

Information you give us

When you complete the consultation request form on this site, we ask for your first and last name, phone number, email address, the service you are interested in, and any message you choose to write. Phone number, email address, and service interest are required; the rest is optional.

Please do not include detailed clinical or medical history in the website form. It is intended only to start a conversation and schedule an appointment. Our staff will collect clinical information through our secure patient intake process.

Information collected automatically

Like most websites, ours records basic technical information when you visit: IP address, browser and device type, pages viewed, referring site, and the date and time of your visit. We use this to keep the site working properly, understand which pages are useful, and protect against abuse and spam.

How we use your information

• Respond to your inquiry and schedule or confirm an appointment
• Verify insurance eligibility and complete patient registration
• Send appointment reminders, and — if you opt in — our monthly practice newsletter
• Operate, secure, and improve the website
• Meet legal, regulatory, and professional recordkeeping obligations

We do not sell your personal information, and we do not share it with advertisers.

Health information and HIPAA

Serenity Health Associates is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Once you become a patient, information about your care is protected health information and is handled according to HIPAA, Massachusetts law, and our Notice of Privacy Practices.

Information submitted through this website before you are established as a patient is treated as confidential and handled with the same care, but general email and web forms are not fully secure channels. Use the phone number below for anything sensitive or urgent.

Calls, text messages, and email

By giving us your phone number, you agree that our office may call or text you about appointments, scheduling, and your care. Message frequency varies, and message and data rates may apply. Reply STOP to any text message to stop receiving them, or HELP for assistance. Consent to receive text messages is not a condition of receiving treatment.

Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent are never shared with third parties.

Email newsletter

We may send an occasional practice newsletter by email — no more than once a month — with clinic news, information about the services we offer, and general mental health resources. You will only receive it if you opt in, and opting in is never a condition of receiving treatment.

Every newsletter includes an unsubscribe link, and you can also ask our office to remove you at any time. Unsubscribing from the newsletter does not affect appointment reminders or messages about your care, which are sent separately. We do not share or sell our email list, and newsletters never include information about your diagnosis or treatment.

Who we share information with

We share information only as needed to provide care and run the practice:

• Service providers who host our website, forms, scheduling, phone, fax, email, and practice management systems, under contracts that require them to protect your information
• Health plans and billing partners, for eligibility, claims, and payment
• Other treating clinicians involved in your care, with your authorization where required
• Government agencies, courts, or others when required by law or to prevent serious harm

Cookies and analytics

This site uses cookies and similar technology to remember your preferences and measure traffic. You can block or delete cookies in your browser settings; some parts of the site may not work as expected if you do. Third-party services embedded on our pages, including our appointment form and patient review widget, may set their own cookies and are governed by their own privacy policies.

Patient reviews

Reviews displayed on this site are submitted by patients through third-party review platforms and are shown as written. We do not add, edit, or remove the content of a review. If a review displayed here contains information you want taken down, contact our office and we will address it promptly.

Data security and retention

We use administrative, physical, and technical safeguards to protect the information we hold, including access controls, encryption in transit, and staff training. No system is perfectly secure, and we cannot guarantee the security of information sent over the internet.

We keep website inquiries only as long as needed to respond to them and meet our legal obligations. Medical records are retained for the period required by Massachusetts law and professional standards.

Your choices and rights

You may ask us to correct your contact information, stop sending you marketing messages, or delete a website inquiry that did not lead to care. As a patient, you also have rights over your medical record — including the right to inspect, copy, and request amendment of it, and to receive an accounting of certain disclosures. Those rights and how to exercise them are described in our Notice of Privacy Practices.

Children

We provide adolescent psychiatry for patients aged 13 to 18, and information about a minor patient is handled under HIPAA and Massachusetts law governing parental access and adolescent confidentiality. This website is not directed to children under 13, and we do not knowingly collect information from them through it.

Links to other sites

Our pages may link to other organizations, including educational materials from treatment manufacturers. We do not control those sites and are not responsible for their content or privacy practices.

Changes to this policy

We may update this policy as our practice, technology, or the law changes. The effective date at the top of this page shows when it was last revised. Material changes will be posted here.
